The vast majority of patients seeking treatment for an eating disorder do not meet full DSM criteria. What are the implications for diagnosis and treatment of eating disorders? What are the broader implications for categorical versus dimensional perspectives on the diagnosis of mental illness?

Since the introduction of the DSM-III-R, syndromes considered 'atypical ', i.e. those that do not fit neatly into other diagnostic criteria, have subsequently been classified as 'not otherwise specified ' (NOS) (APA, 1987). They are considered residual categories and are defined by not what they are, but by what they are not (Keel, Brown, Bodell, 2011). Previously known as 'eating disorder not otherwise specified ' (EDNOS) prior to the DMS-V, 'other specified feeding or eating disorder (OSFED) is the most common diagnosis of eating disorders in outpatient settings (Stice, Marti & Rohde, 2013). This holds major implications for the diagnosis of eating disorders, and in turn even greater implications for their treatment (Fairburn, Cooper, Bohn, O 'connor, Doll & Palmer, 2007). In order to reduce the proportion of OSFED diagnoses, whilst retaining reliability and improving validity, several proposals have been considered in the literature including further relaxing diagnostic criteria, extracting homogenous groups and adding a dimensional aspect to the traditionally categorical DSM (Helzer, Kraemer & Krueger, 2006; Tasca, Maxwell, Bone, Trinneer, Balfour & Bissada, 2012; Fairburn et…
